The April 2021 issue is online and opens with a new and exciting Editorial series: our Global Tour D’Horizon. As a global journal, we are interested to get more insight into the various approaches to vascular care around the globe. Therefore, we have invited many national leaders to describe the main challenges in vascular care together with established solutions from a regional perspective.
George and PC Gupta start off this Editorial series with Vascular Surgery in India – the Challenge and the Promise (read for free). There are many more to come in the coming months.
The second Editorial, by Kitrou et al., appraises New Evidence about Drug Coated Balloons for Dysfunctional Vascular Access which has recently been published in the NEJM. As our authors belong to the pioneers of drug eluting technology in vascular access salvage surgery, they are in a perfect position to put these new findings into perspective (read for free).
The first Editor’s choice paper, by Wang et al., investigates the Mid Term Outcomes of Crossed Limb vs. Standard Limb Configuration in EVAR. Using a propensity score matched comparison, they confirmed that, overall, clinical outcomes of crossed limbs were very similar (with a possible advantage in large aneurysms and tortuous iliacs, but a disadvantage in angulated necks, read for free).

The second Editor’s choice paper, by Gwilym et al., provides a Systematic Review of Wound Adjuncts to Prevent Groin Wound Surgical Site Infection. It seems that only prophylactic closed incision negative pressure wound therapy and subcuticular sutures are effective in preventing groin wound infections (read for free).

The last Editor’s choice paper, by Kim et al., describes the Development and Validation of NZRISK-VASC – a Novel Score from New Zealand to Predict Peri-operative Mortality in Vascular Surgery. This risk calculator (using 10 covariables) is simple and provides good discrimination, but will need external validation and recalibration before widespread use (read for free).
In addition, the April issue features one EJVES Open Access paper: Shen et al. investigated the In Stent Restenosis Process and the Associated Interactions between Matrix Metalloproteinases and Collagen. At least in minipigs, restenosis seems driven by a dysbalance between collagen synthesis and degradation, which could offer a potential target for advanced drug interventions (read for free).
